Description
- Join a dedicated team providing community-based care and advocacy in Bronx, NY.
- This full-time, onsite role involves working with individuals facing complex challenges, blending clinical nursing skills with community support.
- The position requires a Licensed Practical Nurse to work weekdays with flexible start times.
- Licensed Practical Nurse (LPN) in New York State
- Experience with mental health, substance use, homelessness, or criminal justice populations preferred
- Knowledge of community resources and support services
- Strong organizational skills
- Effective communication skills
- Proficiency in computer skills
- Spanish fluency is a plus
- Conduct health assessments and develop care plans
- Support clients in accessing medical care, housing, and community resources
- Collaborate with RNs and psychiatrists to manage medications and coordinate care
- Administer medications, vaccines, and perform phlebotomy
- Monitor vital signs and report medication side effects
- Maintain accurate documentation in the Electronic Health Record
- Provide culturally competent care and participate in crisis intervention
